Fees
Forensic Work: All forensic (legal) evaluations are not covered by health insurance. All payments are due at time of service unless other arrangements are made in advance.

Clinical Work: Dr. Milner is not a participating provider on any health insurance plan. She will provide an insurance claim letter for you to submit for reimbursement of services provided.

Forms
Once you contact Dr. Milner to schedule your first appointment, she will send you forms (such as Informed Consent documents and History Questionnaires) to complete prior to your appointment.
